Oasis Broker Agreements

We will compensate The Broker for the sale of Oasis Health Care plan services to any potential client that signs an agreement with us. The following commission schedule is representative of the commissions paid by Oasis (as long as this Agreement is in force and as long as The Broker continues to be recognized as the broker of record by the account):

Account Size	1st Year	2nd Year	3rd Year plus
200 to 500 employees	10%	7%	4.5%
501 to 1,000 employees	12.5%	8.5%	5.5%
1,001 plus employees	15%	10%	7%

Commissions are paid thirty (30) days after payment is received by Oasis from its accounts.

Oasis may change the above commission schedule from time to time by giving The Broker thirty (30) days prior written notice. No change will effect any current commission arrangement in effect prior to the effective date of the new schedule nor will any change apply to any commission generated by an account agreement that is concluded within ninety (90) days of the effective date.
