Welcome to our Plan Member service site. As an enrolled member in an Oasis Health Care plan, you can perform administrative functions, check plan coverage, ask and get answers to a variety of questions, get a list of PPO providers near you, and provide feedback, all with our convenient online forms.
 
Messages are retrieved every business day between the hours of 8:00 a.m. and 5:00 p.m. Of course you can contact us by telephone simply by calling the toll-free number listed on your Plan ID Card. If you do not have an ID card, please contact your employer's benefits office to obtain this number.
 
Note: These forms should not be used if this is a medical emergency. In the event of an emergency, call the local emergency hotline [911] or go to the nearest emergency facility. Notify us as soon as possible after receiving treatment.
